Tucson Medical Center is pleased to offer you member information via a Web-based version of our electronic medical records called MyChart.

Patient records are lengthy and complex, and MyChart is intended to provide you access to some of your Tucson Medical Center medical records to keep you better informed and more involved in your medical care. Some of your physician's medical records may also be available through MyChart; please contact your physician for information about his or her participation in MyChart, or for access to medical records he or she maintains. For access to your entire Tucson Medical Center record, please contact the Tucson Medical Center Health Information Management Department.

We think it is important for you to know how we handle information we communicate via the Internet. These Terms & Conditions outline our practices and our sensitivity to your right to privacy. We reserve the right to revoke access to MyChart at any time for any reason.

Response to Electronic Communication Tucson Medical Center will make its best effort to provide a timely response to electronic inquiries. In some cases,hospital staff (or your physician's office) may not be immediately available to respond to an electronic inquiry or other communication, so you should allow at least three (3) business days for a response. Emergency or urgent situations requiring immediate attention should not be submitted electronically.

With respect to any electronic communications sent by you, we are only able to respond to such communications based on the information you provide. If insufficient information is provided, we will be unable to provide accurate and reliable services.

Web Site Links - MyChart may offer links to medical Web sites not managed by Tucson Medical Center. These Web site link(s) are for your informational purposes only. Tucson Medical Center does not endorse and has not verified the accuracy of the information in/on these Web sites, and you should not rely on any of the information found on these Web sites for purposes of treatment or diagnosis.

3rd Party Applications - If you as a patient find a 3rd party application (“App”) that you would like to use to access and import your health information, TMCH MyChart provides the ability for you to use this App. To do this, you must establish a MyChart patient portal account because the secure user name and password you set for your MyChart account is used for authentication purposes with any App before allowing access to your health information. TMCH has no control or knowledge as to the safety or privacy of your health information while using this App and you should satisfy yourself as to any privacy settings related to this App. TMCH promotes the protection of your health information to the greatest extent possible and wants to alert you that by using this App, you will assume any and all risk with regard to your health information. TMCH reserves the right to disable any App at TMCH’s sole discretion.

Minor Eligibility and Parent Access - An individual must be age 18 or older to request MyChart access.Parents or guardians of minor children under the age of 18 will have access to the minor child's account. In order to protect the legal privacy rights of older minor children, parents or guardians of minor children will have access to the minor child’s account only with the consent of the child if any of the following apply:

  1. The minor is emancipated. (We must get a copy of the court order.)
  2. A married minor. (We must get a copy of the minor’s marriage certificate.)
  3. Parental rights are terminated. (We must get a copy of the court order.)

Proxy Access - Individuals age 18 or older may request proxy access to another patient's MyChart account by completing the Proxy Access portion of the MyChart Consent Form and submitting it to Tucson Medical Center. Such access will only be granted to individuals who have either (a) the consent of the patient (b) parental rights, legal guardianship, or otherwise have the legal authority to make medical decisions for the MyChart account holder, and only to the extent that the party requesting proxy access has the patient's consent or can demonstrate the legal right to the account holder's medical information. A MyChart account will be activated for both the proxy and the account holder. If the proxy's legal authority to make medical decisions for the account holder changes, the proxy must inform Tucson Medical Center immediately by calling Tucson Medical Center Member Services at (520) 324-1212. Tucson Medical Center reserves the right to revoke proxy access at any time for any reason.

E-mail Privacy - You should be aware that you will be notified via e-mail when there is new medical information to be viewed on MyChart. This means that any person with access to your e-mail will be able to see this notification. This could include your spouse, employer or anyone else that can access your e-mail account. Although no private medical information will be sent, the notification that new medical information is available by accessing MyChart may be information that you would not want others to know. You should take this into account when providing an e-mail address, and you may want to consider setting up a new e-mail account to receive MyChart notifications.

If you send us an e-mail communication, it may be shared with the Tucson Medical Center staff involved in providing your medical care. Your confidential medical information on MyChart will be accessible only to appropriate clinical staff.

Security and Confidentiality - We afford the same degree of confidentiality to medical information stored on MyChart as is given to medical information stored by Tucson Medical Center in any other medium. Tucson Medical Center is committed to protecting the confidentiality of your medical information. We limit Tucson Medical Center employees' access and ability to enter or view information based upon their role in your care. Firewalls, passwords, encryption, and audit trails are further used to safeguard your information. We will track the records released and note the time and date of access each time you access MyChart. We have taken steps to make all information received from our online visitors secure against unauthorized access and use.

This Web site uses encryption software. While our security is very sophisticated, no system can perfectly guard against risks of intentional intrusion or inadvertent disclosure of information. When using TMC MyChart, information will be transmitted over a medium that is beyond the control of TMC and its contractors. YOU HEREBY EXPRESSLY ASSUME THE SOLE RISK OF ANY UNAUTHORIZED DISCLOSURE OR INTENTIONAL INTRUSION, OR OF ANY DELAY, FAILURE, INTERRUPTION, OR CORRUPTION OF DATA OR OTHER INFORMATION TRANSMITTED IN CONNECTION WITH THE USE OF THIS SERVICE. Once the information is received by us, your medical information will be treated as confidential and given the same protection that all other TMC medical records are given.

User names and passwords provide two layers of authentication and are stored in an encrypted database that is isolated from the Internet. As a MyChart user, your role in maintaining the security of your medical information is: 1) Changing your password on a regular basis, and 2) Keeping your login ID and password confidential.
